What you will do
- Hold a named caseload in a defined community area
- Assess and plan care jointly with the person and their family
- Coordinate with therapy services and GP colleagues
What we need from you
- NMC registration as a Registered Nurse (Adult)
- Community or rehabilitation experience
- Right to work in the UK
